| Insurance Information: Payment for health care has become increasingly confusing for all concerned. In order to facilitate the appropriate use of your insurance carrier and avoid financial mishaps, our staff will need to know the most current information about your form of health insurance. This helps us to learn what your insurance covers, what is your responsibility, and the amount of your co-payment or deductible. For our new patients who have been hospitalized, we mail a request for this information. Most insurance companies require us to collect patient co-payments and deductibles on the day of the appointment. Before most appointments, someone from our office will remind you of your appointment and the amount of your visit, co-payment or deductible that is your responsibility. Our staff will notify you before the visit if your insurance plan is one that we are participate physicians. This will allow you to make the appropriate decision when dealing with your health-care. We suggest that you contact your insurance carrier or refer to your plan booklet so that you are aware of which medical services are covered by your plan. |
